Recently, we were designing the home page of a client’s intranet portal built on SharePoint Online. The client wants to display an employee feedback form created using Microsoft Forms on the home page. For this, there is the Microsoft Forms web part.
In this tutorial, I will explain how to use the Microsoft Forms web part in SharePoint Online, especially how to embed Microsoft Forms in a SharePoint page.
Microsoft Forms Web Part
The Forms web part lets you embed a Microsoft Form directly on a SharePoint page.
It is commonly used to collect data, feedback, registrations, surveys, polls, quizzes, etc.
Think of it like placing a form right inside your SharePoint homepage so users can respond without leaving the site.
Add Microsoft Forms Web Part in SharePoint Online
Here, I will show you how to add a Microsoft Forms web part in SharePoint Online. For that, please follow the steps below.
Open the SharePoint site where you want to add the Microsoft Forms web part. Click on Edit at the top right of the site page. Then click on the + circled icon and select a section. Under that section, click on the + circled icon, and then the Search box will appear. Then search for Microsoft Forms and select the Microsoft Forms web part.

Once the Microsoft web part is added, it will appear as shown in the screenshot below. From here, we can add a New Form or an Existing Form.

Once the Microsoft Forms web part is added, we can add a new form to it.
We can add an existing Microsoft Forms form, or we can even add a new form from the Microsoft Forms web part.

Add New Form from Microsoft Forms Web Part
We can add a Form directly from the Microsoft Forms web part in SharePoint Online. For that, please follow the instructions below.
Edit Microsoft Forms Web Part. Under Microsoft Forms, click on New Form. In the right-side properties pane, enter the Form name under New Form. Then click on Create.

Then it will navigate to the Microsoft Forms page as shown in the screenshot. Then click one of the options below “Quick start with“ to start adding the questions. You can also change the style here if you want.

- Then add as many questions as you like. Once all the questions are added, go back to the Microsoft Forms web part edit page. Then click on Republish. Then you can see the Microsoft Form in the Microsoft Form web part, as shown in the screenshot below. Here, I gave only three questions, so it displays only those questions.

In this way, we can add the New form from the Microsoft Forms web part.

Add an Existing Form from the Microsoft Forms Web Part
Here, I will show how to add an existing form from the Microsoft Form web part. Before adding the existing form, we should create a form in Microsoft Forms. Here are the steps to add an existing Form to the Forms web part.
Edit the Microsoft Forms web part, on the properties pane, below the “Form web address“, enter the link of the Microsoft Form. Then choose collect responses. By default, Collect responses is selected. Then click on Ok.

Once you click OK, you can see the form in the Microsoft Forms web part. Here I entered the link of the IT Support Request form.

Once added, we can also edit that form. For that, edit the Microsoft Form.

Edit the Microsoft Form from the Microsoft Forms Web Part
We can also edit a Microsoft Form from the Microsoft Form web part. Please follow the instructions below.
Edit the Microsoft Form web part. On the properties plane, under Microsoft Forms, click on Edit current form.

Then you can edit the form as you want. Once edited, click OK, and the edited form will be displayed in the Microsoft Forms web part. Here, I edited the Employee ID to the Employee name.

Show Form Results from the Microsoft Forms Web part
We can also display form responses in the Microsoft Form web part. Please follow the instructions below.
Edit the Microsoft Forms web part. On the properties panel, select Show form results. Then click on Ok.

Once you click OK, you will see the responses in the Microsoft Forms web part, as shown in the screenshot below.

Conclusion
Microsoft Forms web part helps to display Microsoft Forms and responses directly from the SharePoint Online site. It also helps users submit information quickly without switching between different apps.
