- Microsoft gives a lot of out-of-box web parts, one of the web parts is the SharePoint Microsoft forms web part.
- The Modern SharePoint Microsoft forms web part allows us to create polls, surveys, and quizzes on our SharePoint page.
- We also have an option to show the form results on our modern SharePoint page.
- The Microsoft Forms web part is available on both the team site and communication site but it is not available in the SharePoint Server 2019 and SharePoint mobile app.

How to Add Microsoft Forms web part in Modern SharePoint
Let us see how we can add the Microsoft Forms web part to the modern SharePoint site page,
- Open the modern SharePoint site page then select Edit Page.
- Click + circled icon then add a new Section layout.
- In the search box, enter Microsoft Forms and then select the Microsoft Forms web part.
In the below screenshot, we can see the Microsoft forms web part is added. Either we can create the new form or we can add the existing form to show the form results.

How to create forms on Microsoft forms in Modern SharePoint
Let us see how we can create the forms on the Microsoft Forms web part to the modern SharePoint site page,
There are two ways that we can create forms, they are
- From office 365 Launcher
- From Microsoft forms web Part
From office 365 Launcher
Initially Login to Microsoft Office 365 and select the Launcher icon and click on the Forms as shown below, it will redirect to the Forms page where we can create a new form.
This is the one way that we can create forms in the modern SharePoint.
From Microsoft forms web part
- Directly we can select New form and mention the Form name and the Select Create button redirects to the Microsoft forms where we can add questions to the created form.
- Edit the Microsoft forms web part, and select Add existing form option.
- Select the Go to Microsoft forms( highlighted in Red) in the properties pane of the Microsoft forms web part. It will redirect to the Microsoft forms, where we can create a new form.
- Now select +New Form to create forms on Microsoft forms. We also have an option to change the background or theme for that click on the theme option.
- Rename the Untitled form with your form title area, Select +Add new option to add new questions.
In the title area, you can see that I have changed the title to Training Feedback, and we can also select the data type based on the questions.
If we want to see the Question Suggestions, we can select the flash icon to view them. There is more suggestions option to view the more question suggestions.
While adding the Questions, if we want to make the question to be required, we can enable the required toggle control to yes. We can also have an option to copy and delete the questions.
- We also have an option to insert media for the questions, either we can add images or videos to the questions. We can insert it from one drive, web search, or upload it from the local system.
- Click on the insert media option to insert an image or video.
- Once the image is added, we can edit the image to display as small or large, we also have an option to zoom in or zoom out the image. We can also add alternative text for the image.
- We can also delete the image if we don’t want to display it.
Select the theme option to change the background of the Microsoft form like below
Select the Preview option to preview the Form, either in a Computer view or in a Mobile view. In the below screenshot, I have selected the Mobile Preview option.
- In the Responses tab, we can see the results of the submitted form. We can view the total count of the responses as well.
- We can view the results directly by selecting the view results option. We also have an option to view the results in the Microsoft Excel spreadsheet by selecting the Open in Excel option.
This is another way that we can create and view the responses of the Microsoft form in the modern SharePoint.

Microsoft forms web part missing in SharePoint Online
- The Microsoft Forms web part is available on both the modern SharePoint team site and the communication site.
- The Microsoft Forms web part is not available in the SharePoint Server 2019 and SharePoint mobile app.
- For some users, the Microsoft forms web part feature is missing in the modern SharePoint to fix this issue, you can try to create a different team site page or a communication site page and try to add the Microsoft forms web part.
How to set up forms on Microsoft forms web part in Modern SharePoint
Let us see how we can set the forms on the Microsoft forms web parts in the modern SharePoint,
- Edit the Microsoft forms web part, and select Add existing form option.
- Copy the Form URL and paste the copied URL in the Form web address.
- Now paste the copied URL in the Form web address as shown below, There are two options available in the Microsoft forms web part.
- Either we can collect responses of the from in the modern SharePoint page or we can show the form results in the modern SharePoint Microsoft forms web part.
- Click Republish to save your changes.
In the below screenshot, you can see I have selected the Collect responses options, which display the form Questions to collect the responses from user in our modern SharePoint site page.

To Move the Microsoft forms web part, select the Move web part option available in the Microsoft forms web part toolbox and locate it or move it to the new area.
To Duplicate a form microsoft forms web part select the Duplicate web part option available in the Microsoft forms web part toolbox and make a copy or duplicate of the Microsoft forms web part.
To delete or remove your Microsoft forms web part from the SharePoint Online site, select the Delete web part option available in the Microsoft forms web part toolbox.
